ClearCompany provides state-specific State Tax Withholding forms where available. In the graph below you will find the form name that each state uses for State Tax Withholding, when that form was last updated by the publishing entity, and any known or expected upcoming updates to the form.
ClearCompany clients using these stock forms will have these forms automatically updated both in their Online Forms options and within their New Hire Packet Templates at the time when the updates to the form have been digitized into online fillable form format.
*NOTE: Clients who have opted to have a custom version of a stock online form offered by ClearCompany will NOT have these forms automatically updated and will need to collaborate with their CSM to have these custom forms updated.
ClearCompany regularly checks the publishing entities for updates and revisions. However, if there are new revisions or updates that you are aware of from your regulatory body that are not noted below, please do not hesitate to notify your CSM or support@clearcompany.com with details of the updates needing to be made to a given form.
How to read this table:
- The State column will include the initials of the US State, District, or Territory
- The Form ID column will have the form name according to the publishing entity.
- If a State does not collect State Income Tax, None will be listed for all columns.
- If a State does not use its own form for Tax Withholding, then Federal W4 will be listed. Updates around US Federal forms can be found here.
- Date of Last Update refers to the year that the publishing entity last published a new or revised form.

Upcoming Updates refers to any expected upcoming updates to the form.
- When an update is expected, but no details have been provided by the publishing entity, TBD will be used until more information has been provided.
- An estimated date of completion will be provided in this column based on when ClearCompany anticipates updating client accounts with an updated or revised form.
If you notice that a form update or revision was missed or a mistake in one of the forms, please submit a request to support@clearcompany.com with the name of the form and a brief note of the needed update or revision.
