Minor authorization forms: when they’re required

Minor applicants: A parent or guardian must sign a Minor Disclosure & Authorization before screening can proceed.

When required: Any background check for an applicant under the age of majority (as defined by the applicable jurisdiction) requires a parent/guardian-signed minor authorization form.

Notes:

  • If the required minor authorization is not received, the report will be canceled. You may re-send the signed form to restart processing.
  • You may also be prompted to complete an additional questionnaire to finish your background check; follow any follow-up instructions you receive.

Tips

  • Ensure all required fields are completed, and signatures are clear.
  • Keep a copy of the signed form for your records.
  • If a submission was sent to the wrong address, resend it to the designated email or upload it directly to the profile to avoid delays.
